Your privacy. The PDF and your answers stay in your browser; we never receive them. If you use the Speak button, your browser's speech service may send your voice to the browser maker to turn it into text.
2. Choose the PDF you downloaded
Use a form from the free library. The form must have fillable fields; many Florida forms do.
Optional: paste the form's official instructions
Paste text from the issuing agency's instruction sheet. Each field then gets a button that shows only sentences copied word for word from what you pasted.
3. Enter your answers
Prefer to talk instead of type?
- Tap Speak beside a text box, say your answer, then tap Stop. Check the words and fix anything it misheard.
- Or use your device's own dictation: on iPhone or Android tap the microphone on the keyboard; on Windows press the Windows key + H; on a Mac choose Edit, then Start Dictation.
- Say numbers and dates the way you would read them aloud, for example "October fifth nineteen eighty".
Leave signature and date-of-signing lines blank if a notary will watch you sign. For help deciding what to write, contact the court's Self-Help program (opens in a new tab) or a Florida attorney; this tool cannot answer those questions.
